HOCKEY CANADA ID & MEDICAL FORM

You will need to know your daughter's Hockey Canada ID Number which you can find by logging into your hockey Canada Account or by checking a prior years registration receipt that would have been emailed to you in the past. If you search "noreply@hockeycanada.ca" in your email and open the receipt attachment, beside your daughter's name will be her birth year and her Hockey Canada ID Number.

You must submit your daughter's Hockey Canada Medical Sheet to your team manager this season. The form is a fillable PDF file that you can download below or during the registration process. All players need to have these medical forms filled out before they are on the ice.
